女性不孕症及子宫内膜异位症患者腹腔液一氧化氮水平的变化及意义

【摘要】
目的:探讨腹腔液一氧化氮水平与不明原因的女性不孕症及子宫内膜异位症的关系。
方法:测定25例不明原因的不孕症患者(不孕症组)、38 例子宫内膜异位症患者(内异症组)、18例子宫肌瘤和2例卵巢囊肿患者(对照组)腹腔液一氧化氮代谢物(硝酸盐和亚硝酸盐)浓度,比较各组间一氧化氮代谢物水平的差异,观察一氧化氮代谢物水平与子宫内膜异位症期别及是否合并不孕症的关系。
结果:不孕症组腹腔液一氧化氮代谢物为42.02±12.98 (±s,mmol/L),内异症组为41.753±16.42,均显著高于对照组(33.96±13.07,P均<0.05);不孕症组和内异症组间无显著性差异(P>0.5)。不同期别的子宫内膜异位症患者腹腔液一氧化氮水平无显著性差异(P>0.5),合并与不合并不孕症的子宫内膜异位症患者腹腔液一氧化氮水平无显著性差异(P>0.5)。
结论:不明原因的不孕症和子宫内膜异位症患者腹腔液一氧化氮水平显著增高。这一变化可能与不明原因的不孕症、子宫内膜异位症导致的不孕症的发生以及子宫内膜异位症的进展有关。

Evaluation of nitric oxide metabolite in peritoneal fluid from women with idiopathic infertility and patients with endometriosis ( Dong Min-Yue, Cheng Qi, Hao Min,Shi Yi-Fu. Obstetrics and Gynecology Hospital, School of Medicine, Zhejiang University, Hangzhou, 310006).
【Abstract】 Objectives: To evaluate the relationship between nitric oxide (NO) level and idiopathic infertility, endometriosis. Methods: 25 women with idiopathic infertility and 38 patients with endometriosis were recruited, 18 cases of myomata and 2 cases with ovarian cyst were used as controls. Peritoneal fluid samples were aspirated from pouch of Douglas during laparoscopy or laparotomy. Products of NO metablism, i.e. nitrite and nitrate, in peritoneal were determined. Results: Concentrations of products of NO metabolism in infertile women(42.02±12.98 mmol/L) and patients with endometriosis (41.75±16.42 mmol/L) were significantly higher than that in controls (33.96±13.07, P<0.05 for both). No significant difference of NO levels in peritoneal fluid was found between infertile women and patients with endometriosis(P>0.5). Peritoneal fluid levels of NO products were comparable in patients with endometriosis at different stages(P>0.5). No significant difference of NO levels in peritoneal fluid was found between patients of endometriosis with and without infertility(P>0.5). Conclusion: Nitric oxide metabolites increased significantly in peritoneal from women with idiopathic infertility and patients with endometriosis. It is supposed that nitric oxide may play a role in pathogenesis of idiopathic and endometriosis-associated infertility, and may be involved in disease process of endometriosis.
